Consent / Event instructions:
- The consent form link will take you to the Primary Health Consent Form to complete.
**Click on the "OTHER REGISTRATION" link and enter your name, address, email, mobile phone, and other information.
- You will receive an email and/or text confirmation once you have completed your consent.
- When you arrive at the flu clinic, open your confirmation text or email and show it to staff at the entrance.
- Next, follow the link embedded in the confirmation message to get your bar code at the bottom of the page.
- Medical Staff will scan the barcode, confirm your identity (bring your ID), and give you your shot.
